Meet Our Team

Loving care is delivered by a team of professional who address the patient's medical, physical.,
emotional and spiritual needs. Our care team is compromised of physicians, nurses: hospice aide,
social worker, chaplain, volunteer and bereavement counselors.

All patients in hospice is under the care of a hospice physician who closely monitors the progression of the patients illness, developing a plan of care to meet each patient's specific needs prescribes appropriate medications and coordinates care with other members of the team.

Visiting Physician

Our Nurses have specialized training in pain and symptom management, who provide hands-on patent care They evaluate patient and family needs, as a Skilled listeners, they coordinate the plan of care with the care team, and guide education for patients and their loved ones. Hospice nurses monitor, assess, and handle everyday medical needs. From making sure the patient takes prescribed pain medication, to setting up medical equipment, and taking daily temperature or blood pressure readings.
These nurses also help family members understand how to better care for the hospice patient. Hospice nurses comfort the family while also teaching them how to take the best care of their loved one.

Social workers coordinate the logistics of the patient's care providing emotional and psychosocial support to the patient and family. They are working with insurance companies funeral planning or other tasks. Social workers are always available for direct counseling and arranges referrals to community agencies as necessary.

Chaplain

Chaplains are available to address the spiritual issues that often arise as a patient nears death. Regardless of a person's beliefs or religious traditions. The hospice chaplain is there for the patient and the family, honoring and supporting the cultural  traditions and values they hold dear, provide direct counseling, as well as providing bereavement support. When requested, the hospice chaplain works with the patient’s specified clergy

Hospice Aides are the heart of care, certified nursing assistants who provide personal care to the patient, such as bathing, dressing, and light housekeeping. Our hospice aides are known for building warm relationships with patients and families as they bring personal care, comfort and emotional support.

The bereavement specialist addresses both anticipatory grief and loss after death. Bereavement counselors meet with family members after the hospice patient's death to help them through the grief process. Bereavement services are available up to 13 months after the patient's passing. These include consistent contact, support groups, grief education, and one-on-one visits.. Hospice is always available to those who've experienced the death of a loved one.
